What do you need to do?

Before an activity

Identify hazards, assess risks, agree responsibilities and make an emergency plan. Use the venue’s procedures and your discipline’s requirements.

If something happens

Make people safe and get appropriate help first. Record the facts, notify the responsible person and use the right reporting route.

After an incident

Review what contributed, record actions and follow through. Share useful learning without sharing someone’s private information.

Use the right reporting route

An incident report records a safety event. Be Heard is for concerns and complaints, including behaviour or safeguarding. Neither replaces emergency assistance or any required regulator notification.

For immediate danger call 111. For work-related events, the responsible organisation should check WorkSafe’s notification guidance; submitting to the hub does not notify WorkSafe.
